A current MHSAA Physical Form must be on file in the Athletic Office in order for students to participate in athletics. Physicals are considered current if dated on or after April 15th of the previous school year (April 15, 2019 for the 2019-2020 school year). Students and their parent/guardian should complete the medical history section. The form must be signed and dated by the examiner. In addition, the student and parent/guardian must also sign and date all required areas of the form.

Any student who received a physical for the 2019-2020 school year (dated on or after April 15, 2019) can submit this form in lieu of a new physical. If a physical was submitted to the athletic office for the 2019-2020 school year, it will remain on file, and all that will need to be turned in to fulfill the physical requirement for 2020-2020 is the Sports Health Questionnaire. Students that had a physical on or after April 15, 2019, may turn that physical and the Sports Health Questionnaire in to fulfill the physical requirement for participation in athletics. Any student who wishes to participate in athletics for the 2019-2020 school year but did not have a physical completed on or after April 15, 2019, must visit a health care provider to have a physical. That physical will then need to be turned in to the athletic office.

The Athletic Handbook covers all policies for participating in athletics. The last page of the document is the Athletic Handbook Acknowledgement Form, which must be signed by a student and parent/guardian. The signed form must be on file in the athletic office in order to participate in any sport.
